Immunization Requirements
New York State public health law requires documentation of immunization for Measles, Mumps and Rubella (MMR) and Meningococcal Meningitis. New students must provide documentation of these two immunization requirements upon admission or at least 30 days prior to their scheduled registration date. For this reason, a hold is placed on registration for all incoming students. The Columbia Health office will place a hold on your account if they do not receive your certification.
MEASLES, MUMPS AND RUBELLA (MMR)
All students must document their immunity to measles, mumps and rubella. You may find additional information on providing this documentation on the Columbia Health website.
Immunization records that have been completed in a language other than English must be either translated and notarized, or accompanied by an official translation. Alternatively, students and their health care providers can complete the Columbia University MMR Form and submit the original to the Immunization Office at the address noted below.

MENINGOCOCCAL MENINGITIS VACCINE
Public health law requires that students receive information from their institutions about meningococcal meningitis and the vaccine that protects against most strains of the disease. Columbia students must make an informed decision about being vaccinated and certify their decision online. The process takes about three minutes to complete.
